Well gee that one is a toughie is it Mushroom Hill? Hehehe No no of course the intro that leads into City Escape. Technically not even a level persay as a cut scene. So I guess the best way to answer it is "The opening to the Hero Story of Sonic Adventure 2"
This one may be tricky depending on if you've heard this phrasing before. The clue is something known to many as the Barrel of Death. -

Ooooh, that's an interesting one. The references to lava automatically makes one think of things like the Lava Reef Zone, or Crisis City if you want to look more modern. But the 'many faces of the evil doctor' makes the whole thing a bit more specific... and I'm guessing is referring to something a little more literal than you'd normally interpret that phrase. Add the mention of getting the Emeralds first to get to the boss...
