Kingdom Hearts(all of them XD)
Huh, never considered Kingdom Hearts of all games to be a Hack n' Slash. Then again, there's an awful lot of button-smashing and strategy in those games...

Well, it's not for PC, but I do recommend Star Ocean: The Last Hope.

The exploration is standard, but after an enemy encounter, it basically turns into a hack-n-slash.

I remember playing this game, so I can vouch for this. It's pretty much a hack-n-slash whenever enemies at any point pop up in this game.

HARD WORK AND GUTS.
Odin Sphere, Chantelise, Zelda... slower paced hack 'n slash RPGs are the ones I like. Taking your time, thinking about how you might take on your enemies, unlike games like Ninja Gaiden, where you hurl yourself in, get killed and THEN strategize.

How dare none of you mention the Tales series? :V It's a perfect blend of RPGs with strategical hack-and-slashing and stuff. None is for the PC though. Tales of Vesperia(360), Tales of Symphonia(GC), Tales of the Abyss(PS2 and later 3DS), and the upcoming Tales of Graces f for the PS3 are among the few.

I only ever played the Mythology series on PSP. It was fun to begin with, but then it started taunting me with all the awesome advanced classes that I couldn't get unless I grinded those points for hours. I got bored midway, still stuck as a thief, and just gave up after a while. I might retry if Mythology 3 gets a translation though, the gameplay WAS fun for the time I did play.

I still need to play Odin Sphere, it looks like a really good game to play. And then the art... Oh, the art is beautiful! As for Ninja Gaiden, that game is simply brutal to people who try to rush the game.

Trouble is, Ninja Gaiden presents no form of retreat and foes spawn on the fly, giving you little option other than to rush at foes and either hope you die quick enough or just die slowly, before you learn what the foes do and how's best to fight them.

Though seriously dude... get Odin Sphere. Worth it. If it's $50. Still worth it.

As much as I respect Diablo for pushing forward the whole Isometric Dungeon Crawler sub-genre, I couldn't get to grips with the gameplay or stylistics.
